Complete this form to confirm that all committee members have reviewed the student’s thesis or dissertation document and approve it as ready for defense. Once approved, the document will be made available to the department for public display, after which the student may schedule the defense date and room.
The thesis or dissertation document must be placed on public display at least five (5) business days before the scheduled defense. To initiate this process, the committee chair enters the names of all committee members, who will each receive a DocuSign link to sign. After all members have signed, the committee chair signs again to indicate whether the document is ready for defense. The graduate student signs last to complete the process. The committee chair should then forward the approved thesis or dissertation document to the Graduate Coordinator for public display.
